The following is general advice to all, not specificaly to Shrink (who it applies to).
If you want to improve your body, you need to rebuild the whole wall, not patch one of the holes in it. From the pictures I've seen of Shrink, that's going to be a task requiring a very powerful will and a good bit of time. It won't be a lot of fun at first, but it's necessary.
It means: a healthier diet, exercising, cutting out the cigarettes, alcohol, and drugs, and learning to stress less. As it is, you're a walking time bomb. If you're smoking, drinking, and doing drugs you've probably already done some irreversible damage in terms of years lost (although not a lot if you start correcting it now), and you probably won't make it to 50 (perhaps 55) without a major problem (or death) intervening.
It doesn't get better if you do nothing about it. Once the major problem happens, it's really too late to do much more than try to prevent it from happening again, and you'll either have to compromise your lifestyle or continue ignoring it and die. Neither way is any fun. In other words, if you want to prevent more problems later, act now.
