Kahnawakee Gaming Commision, based outside of Montreal on an Indian Reserve, licences a lot of casinos. Supposedly, they host a lot of servers there too.

I doubt it comes close to CR. SIA, Wagerstreet, Bodog and ? There's probably 200 in CR. England would be my guess for #2.
