What I was trying to ask is, does anyone have an authorization form for making withdrawals? VIP.com says I must fill one out and fax it to them. I have never heard of such a thing, and although I have asked them repeatedly since Sunday (countless e-mails, a live chat, a phone call), no one there will send me one.
Perhaps it is a standard form other sports books use too?

usually this type of form must come from the sportsbook themselves.... sometimes it is needed when one makes a deposit via Credit Card.
Have you tried contacting them via Live chat?

Finally this aft they send me this inside the body of an e-mail:
"Customer Purchase Agreement
I, _____________, have read and agree to the Terms and Conditions posted on the Web page associated with my gaming account ______________. I authorize transactions made on my Credit/Debit Card made at Virtual Exchange Inc – VIP.com and agree that the transactions will be debited from my Credit/Debit Card and will be posted on my statements accordingly.

Customer Signature: ______________________
