Anyone request a withdrawal with Bodog when using credit card deposit?

Over the past 2 months, I made 4 deposits to Bodog using 4 different credit cards. I ended up losing the first 3 deposits on bets. But on the 4th deposit, I managed to turn it close to $5000. Now I want to request a withdrawal.

Do I have to fax them copies of all 4 credit cards & statements or just the last cc that I won with?

The address on my driver's license does not match the one in my Bodog file because it's 5 years old and I have moved since. I have to renew it in June at which time I will correct the address. So should I wait till June to request withdrawal or is it OK right now?

Would it be alright to request the $5000 via Western Union or would this draw a red flag?????

This is the e-mail I received after requesting a withdrawal a while back. I had to do the same thing with all credit cards that I had ever used to deposit. I think your driver's license has to match with the addresses on your credit cards. Maybe you should just renew your license a little earlier.

Thank you for your inquiry regarding the Credit Card Verification Form (CCVF).
We require the CCVF in order to verify that
the person who is using the credit card is the actual cardholder. More
importantly, when issuing payouts to our clients, we
want to verify we are paying the funds to the correct person. The CCVF does
exactly what it is titled in that it verifies
you are the credit card holder. The security risk involved in using credit
cards to deposit is that we mail checks as a form
of payout instead of crediting your card with the funds. Furthermore, the
Credit Card verification form acts as a security
measure to guard against fraudulent transactions.

This requirement is applied to all our clients who deposit with credit cards.
Once we receive this information we will re-
submit your payout request to our Payouts Department for processing. Provided
you do not use additional cards, you will not
be required to send in this form again.

To access the forms, please log into your Bodog account and visit our “Help”
section, located at the top right corner of the
screen, followed by “Financial” then “Verification Forms”. On this page
you will find the heading “Credit Card Verification
Form”, followed by the “Click here to view the Credit Card Verification
Form” button. Once you click on this link, the Credit
Card Verification Form will appear in another screen. You may send the from to
documentation@bodog.com .

Did you try faxing over only the credit card that you won with and they told you that you have to fax every cc that you ever used?

One of my deposits was using a bank debit card (with Visa logo), which means there is no "credit card statement" for..... so anyone know how to deal with this?

I never had to actually fax in a credit card statement. I think I had to just to put my card underneath a sheet of paper and use a pencil to get an imprint of the front of the card and then sign the verification form. I just scanned that and sent in a scanned copy of my license. However, I did have to do the same thing for all the cards that I used. That was all, and the withdrawal through WU came quick after that.
