Koufax is one, the other three are oldtimers Lefty Williams and Ed Ciccotte (both victims of the 1919 Chicago Black Sox career ending suspensions). The final one is strange because he also was a rookie when he won 20 games in his final year in MLB.

The answer is The Denver Broncos' safety John Lynch. He threw out the ceremonial first pitch at the Florida Marlins first home game. Lynch has either a plaque or as some people say the hat he wore that day hanging on a wall at the Baseball Hall of Fame in Cooperstown, Ohio.
Hat - plaque - kinda of splitting hairs in that regard as the principle part of the question is what NFL Player is in the Baseball Hall Of Fame.
