Only know this because I witnessed it first hand thanks to the Red Zone Channel (which is awesome by the way).
I'm sure things like this have gone on for years (Ray Lewis is good for this on every tackle almost the way he throws elbows, and free falls on players), but when it is very obvious like this I'm surprised the league hasn't intervened.
The two examples I saw were Stephen Jackson & Justin Forsett.
Jackson was bottled up in a pack and one Washington defender grabbed his leg, lifted it up and bent it sideways stretching his groin. Jackson may now miss time.
Forsett was running in the open field and was tackled by one defender by the legs but the defender twisted his ankle around while tucking it under his arm. Forsett limped off the field and missed some time but returned later.
Two very obvious attempts IMO to try and injure guys. While I am surprised no players ever complain about stuff like this, I totally understand as I am sure they would be called soft,etc.........
