TRENTON, N.J. (AP) — Johnson & Johnson said Thursday that it's reducing the maximum daily dose of its Extra Strength Tylenol pain reliever to lower risk of accidental overdose from acetaminophen, its active ingredient and the top cause of liver failure.
The company's McNeil Consumer Healthcare Division said the change affects Extra Strength Tylenol sold in the U.S. — one of many products in short supply in stores due to a string of recalls.
Starting sometime this fall, labels on Extra Strength Tylenol packages will now list the maximum daily dose as six pills, or a total of 3,000 milligrams, down from eight pills a day, or 4,000 milligrams. Beginning next year, McNeil will also reduce the maximum daily dose for its Regular Strength Tylenol and other adult pain relievers containing acetaminophen, the most widely used pain killer in the country.
Besides Tylenol, acetaminophen is the active ingredient in the prescription painkillers Percocet and Vicodin and in some nonprescription pain relievers, including NyQuil and some Sudafed products. It's found in thousands of medicines taken for headaches, fever, sore throats and chronic pain.
But people taking multiple medicines at once don't always realize how much acetaminophen they are ingesting, partly because prescription drug labels often list it under the abbreviation "APAP."
Two years ago, a panel of advisers to the Food and Drug Administration called for sweeping restrictions to prevent accidental fatal overdoses of acetaminophen.
Then in January, the FDA said it would cap the amount of acetaminophen in Vicodin, Percocet and other prescription pain killers at 325 milligrams per capsule — just under half the 700 milligram maximum of some products on the market then. The agency also said it was working with pharmacies and other medical groups to develop standard labeling for acetaminophen.
"Acetaminophen is safe when used as directed," Dr. Edwin Kuffner, McNeil's head of over-the-counter medical affairs, said in a statement. "McNeil is revising its labels for products containing acetaminophen in an attempt to decrease the likelihood of accidental overdosing."
Excessive use of acetaminophen can cause liver damage. In the U.S., it's blamed for about 200 fatal overdoses and sends 56,000 people to the emergency room each year.
McNeil spokeswoman Bonnie Jacobs said other makers of pain relievers are likely to make similar changes to their product labels.
Extra Strength Tylenol is manufactured at a J&J factory in Las Piedras, Puerto Rico, where production has been decreased for months because the FDA, concerned about manufacturing and quality problems, is requiring additional reviews and approvals before medicines can be shipped. J&J said shipments of Extra Strength Tylenol should ramp up in the latter part of this year and throughout next year.
Las Piedras is one of three factories implicated in most of the 25 Johnson & Johnson recalls since September 2009, involving tens of millions of bottles of Tylenol and other nonprescription drugs made by McNeil. Several prescription drugs, hip implants and contact lenses made by other J&J subsidiaries also have been recalled.
The recalls, for quality problems ranging from metal shavings and improper levels of active ingredients in some medicines to packaging with a nauseating odor, resulted in a consent decree between McNeil and the FDA this spring.
As a result, Las Piedras and a second factory, in Lancaster, Pa., are under additional scrutiny. The third factory, in Fort Washington, Pa., made children's medicines such as liquid Tylenol. It has been closed since April 2010 and is being gutted and completely rebuilt.
Jacobs said the label changes are not related to the recalls.

What the pharm companies out to do is take Tylenol completely out of prescription pain pills. Unbelievable that 56,000 people go to the E.R. every year because of just regular Tylenol & get an over dose.

I used extra strength tylenol for years till i started to feel sick when ingesting it.
Since about a year ago i'm on liquid advil instead, been taking it almost daily for
some time, and pleased with the results. For headaches i also use homeopathic
and herbal pills and a creme that is applied to the nose & temples.

I used extra strength tylenol for years till i started to feel sick when ingesting it.
Since about a year ago i'm on liquid advil instead, been taking it almost daily for
some time, and pleased with the results. For headaches i also use homeopathic
and herbal pills and a creme that is applied to the nose & temples.

I used extra strength Tylenol for years as well and now suffer from liver disease as the article indicates. I don't drink so that has had no bearing on the disease. Over the counter Advil is a motrin based product and in moderation a much better product for pain. Just be sure to eat it with food in order to avoid any irritation to your stomach. Be well my friend.

Too bad codeine phosphate isnt available over the counter, it would definitely be a better alternative to Tylenol. Tylenol is okay now & then but destroys so many livers.

Thanks for this Ted. I wasn't aware Advil should be taken with food.

I was previously diagnosed with nonviral hepatitus, but the doctors could never figure out what was happening; they called it a mystery after numerous blood tests & an MRI. I guess they were looking for a cause, took me off all kinds of foods, vitamins & meds, wanted to stick a needle in the organ & take a sample of it, but i wasn't ready for that. At times the liver hurts as, i guess, it is being burned up from alcohol over use, although i've heard it can grow back again {unlike if a limb is lost}. When trying to sleep my organs have been hurting for months when i try to lay on my sides but on the back is ok. So in the last week i've been cutting the booze to a minimum, 2 drinks or none at all, to see if that will help them recover.

I've never heard of codeine phosphate before. My doctor prescribed me T3's {a tylenol & codeine mix} for something like stomach cramps.
A couple years later she wouldn't give me any more saying they were bad for the liver. I was seeing a liver specialist at the time. One time
i had a bad reaction from a T3, was getting dizzy etc, & called an ambulance.

Codeine Phosphate is pure codeine w/o the Tylenol crap. In the U.K., you can buy these easily. They come in 30mg & 60mg tablets & work pretty good if you dont have a tolerance. Thing is that codeine relief doesnt last as long as say, Vicodin. Tylenol 3's are okay for wisdom teeth pulling & what not but when you have serious pain problems, they are worthless imo.

Has your doctor ever given you any anti-inflammatory drugs? They work wonders for pain.

or anything with codeine in it legally.

after a large lady was on my chest for too long & caused some damage. That was
around the time i switched from tylenol.

Advil is an anti inflammatory. BTW, the active ingredient in Advil is Ibuprofen which is the same as Motrin.

Yes Advil is an anti-inflammatory. If your doctor can prescribe you Naproxen which is pretty much prescription Alleve. I have been prescribed Nabumetone 500mg tabs & they work great..............they also work like a muscle relaxant which give me a good nights sleep.

Also, I hate to ask but why was a large woman sitting on your chest? I personally would never have a woman over 130 pounds lay on me but that just me, everyone is different..................:)

in my country, even opium is easily accessed, because most of farmers here grow those on their backyard.

She was maybe 170 pounds with big breasts banging me while putting her full weight on my chest for about half an hour. While this was happening my big head was telling me it hurts, but my little head said he liked it. You know who won that argument. Thereafter the pains near my heart had me concerned, but luckily there were no broken ribs, just an injured muscle. Usually the girls are more like 100 lbs and it results in the stomach cramps referred to earlier.
